Adult membership plan

• enVisage Membership Plus is underwritten by Denplan. Terms and conditions to be read in conjunction with Denplan's terms and condition

• All benefits apply to up to date membership and maintained membership
  Denplan charge a one off joining fee equal to one months membership fee which will be debited with the first months membership fee

• Membership will start from the 1st of the next calendar month. Applications filled in towards the end of the month may have the direct debit delayed until the following month due to banking regulations. In this situation the first direct debit may be the joining fee and two months membership fees together

• Tooth whitening for life
  • Applies to Home-based tooth whitening system
  • A clinical assessment for suitability is neede
  • Patient compliance with instructions is required
  • The first whitening procedure is charged at the members discounted fee
  • It will be carried out by a hygienist or therapist as prescribed by your dentist
  • Patients must be 18 or over
  • A consent form must be agreed to and signed
  • You will then receive 3 syringes of whitening gel each yea

• Free Braun Oral B electric toothbrush
  • The brush will be issued after 3 months membership
  • The toothbrush supplied may vary

• Membership discount on private fees
  • 20% reduction on restorations, periodontal plans, consultations, extractions, x-ray pictures, hygienist visits and root canal treatment
  • 10% reduction on Crowns, Bridges, Veneers, Implants, Intravenous sedation, Dentures and related services including laboratory costs
  • The dentist will use their normal laboratory
  • Specialist dentistry excluded
  • Does not apply to treatment plans written before 19th October 2009

• 3 year restorative guarantee
  • Refer to separate terms and condition

• No cash alternative. Not transferable. We cannot waive the callout fee of any other dental practice you may use for an emergency away from home. We cannot waive the call-out fee of any other dental practice you may use for an emergency away from home. If you use one of the dental practices on our emergency rota, there will be a minimal call out fee, currently £15
